ATHLETIC CLEARANCE

In order to participate in athletics at RMHS you must first be athletically cleared which involves the following:

*Creating an account for every sport your student will participate in at https://www.athleticclearance.com/

*Complete steps 1-4. Step 5 is optional.

*Must have medical insurance & provide name and ID or policy number on medical card

*Must provide the students doctors name and phone number

*Athletic Physical Form Athletic Physical & Parent Consent Form in English and Spanish

*Physical-upload your student’s current completed physical to the account.

*Transportation Form OUHSD Transportation Form in English and Spanish

*Transportation Form-Complete and upload to student’s athletic clearance account under “Upload Additional Form” or you can email it to olivia.ramirez@oxnardunion.org or bring in a hard copy to our athletic secretary, Olivia Ramirez. You only have to complete the insurance section at the bottom of the form if your student has a valid driver’s license and you want him/her to be able to drive themselves to athletic events or if you would like to assist in transporting athletes to pre-season games or tournaments that the school doesn’t provide transportation to.
